In this section, you'll guide your client through a series of questions about his or her health, designed to help you provide a more accurate quote.

The Underwriting screen is Step 4 in the Application process.

If you selected the Referral process at the beginning of the quote path, you will not see this section. When you submit the application for processing, our application fulfillment partners will reach out to your client on your behalf and complete the discovery and underwriting portions of the process. Here’s what this step of the quote path looks like. Keep in mind that you’ll always have a note field available on the right side of your screen; we’re not showing it in the screenshots below to give you a closer look at the quote process. Blood Pressure Information

  • Have you ever been diagnosed with or treated for high blood pressure? Select yes or no. If yes, the system will prompt you to ask about the client's systolic pressure, diastolic pressure, last treatment, and length of time he or she has been on medication.

Cholesterol Information

  • Have you ever been diagnosed with or treated for high cholesterol? Select yes or no. If yes, the system will prompt you to ask about the client's cholesterol level, HDL ratio, last treatment, and length of time he or she has been on medication.

Health History

  • Have you ever been diagnosed with or treated for any of the following health conditions? Here, you should check off any major medical conditions your client has. Examples include diabetes, cancer, cardiovascular conditions, mental/nervous conditions, and more. In most cases, the Insureio health screening tool can immediately evaluate one major medical condition. Selecting two or more conditions will require evaluation by our in-house underwriter or your brokerage director to receive accurate health rating recommendations.

Family History

  • Please indicate the total number of immediate family members (parents or siblings), if any, who have contracted cardiovascular disease (heart attacks or strokes), cancer, diabetes, or kidney disease prior to age 70. Type in the client's answer, or click the up and down arrow buttons in the right side of the entry box. If you type in a number greater than 0, Insureio will ask for more information - whether the individual affected was your client's parent, the age they contracted the illness, the age of death, and a checklist of other ailments that family member may have had. You can also click "Remove this Record" if you arrived here by mistake.

Driving History Information

  • Have you had your license suspended or revoked or had more than one ticket or accident in the past 5 years? Click a radio button to indicate whether your client has had their license suspended or revoked or had more than one ticket/accident in the past five years. If yes, Insureio will prompt you to ask a series of follow-up questions about your client's driving history.

Avocation Information

  • Do you participate in any hazardous activities like racing or motor sports, hang gliding, piloting, rock climbing, scuba diving, or skydiving? Click a radio button to indicate whether your client participates in any hazardous activities. If yes, Insureio will provide a text box for you to type in the details and dates of the activity.

Bankruptcy

  • Have you ever declared bankruptcy? Click a radio button to indicate whether your client has ever declared bankruptcy. If yes, Insureio will prompt you to ask follow-up questions about the date and whether the bankruptcy has been discharged.

Criminal History Information

  • Are you currently on parole or probation OR have you ever been convicted of a misdemeanor or felony? Click a radio button to indicate whether your client is currently on parole or probation or has ever been convicted of a misdemeanor or felony. If yes, Insureio will provide a text box for you to type in the details and dates of the incident(s).

Foreign Travel

  • In the past two years or next two years, have you travelled or do you plan on traveling outside of the US? Click a radio button to indicate whether your client has traveled or plans on traveling outside the US in the past two years. If yes, Insureio will prompt you to ask where, when, and the anticipated length of stay.

Residency

  • Are you a US citizen?Click a radio button to indicate "yes" or "no" for your client's US citizenship. If you click "no," Insureio will prompt you to select a citizenship status: permanent resident, visa, or other.
